Policy for the Treatment of a Minor
It is the policy of Lake Eye Associates that all patients under sixteen years old MUST be accompanied by a parent or guardian when being seen for visits with the physicians or other types of appointments in the clinic.

Policies Regarding Minors
Children 0-16 Years
If a parent/guardian is absolutely unable to accompany the child, then another authorized and responsible adult should accompany them. The parent/guardian will be required to complete a Permission to Accompany a Minor Form. Additionally, for a child under sixteen to be seen the parent must also agree to be available to be contacted by phone during the exam by the provider. If a parent cannot be contacted during the visit the child may not be seen.
Children 16-18 Years
Patients between 16-18 years may be seen without a parent for a follow up appointment. However, the parent/guardian must still complete a Permission to Treat a Minor Form. If permission from a parent cannot be obtained, the child cannot be seen. Additionally, the parent must also agree to be available to be contacted by phone during the exam by the provider. If a parent cannot be contacted during the visit the child may not be seen.
Patients Ages 18+
Patients who are 18 and older have a right to privacy which is covered by HIPAA laws. That means that we cannot release any information to parents or family members without a signed release by the patient, even if the patient is on their insurance or the parent/guardian is paying the bill. Financial responsibility does not grant rights to personal/protected information.
We welcome Medicare patients and participate in most health care plans. As we are continuously adding plans, please contact your insurance provider to verify plan participation.
You are responsible for your Lake Eye Associates charges. Payment is expected at the time services are rendered. For your convenience we accept Master Card, Visa, American Express, Discover and Care Credit as well as checks and cash. You may also submit payment by clicking on the Pay Your Bill link on the top menu bar. Please make any special payment arrangements in advance. Eye glasses require a 1/2 down payment when ordered and the remainder of the charge is due when they are picked up.
Lake Eye Associates is prepared to file insurance claims for charges incurred at our clinic. In many cases your medical or eye care insurance will cover these charges. Please be sure to notify us of any insurance plans or changes you may have when you make your appointment.
